Winchester Rehabilitation & Nursing Center, a Salter Healthcare facility, currently is seeking an MDS Coordinator. We provide rehabilitation programs, post-acute short-term and long-term health care, skilled nursing and hospice care as well as respite services.
The MDS Coordinator will conduct and coordinate the development and completion of the resident assessment in accordance with current rules, regulations and guidelines that govern the resident assessment, including the implementation of CAA’s. Participate in facility surveys made by authorized government agencies. The MDS Coordinator will also work with our interdisciplinary care team in developing a comprehensive resident assessment and care plan for each resident.
Requirements include:
Licensed RN in MA with 2 years of MDS experience and previous care planning experience.
