Holden Rehabilitation and Skilled Nursing Center is now looking for a full time Case Manager/MDS Coordinator to join their team of caring professionals! Holden Rehabilitation is a beautiful 123 bed skilled nursing facility providing short term rehab, long term care, respite stays and is home to the Wachusett Respiratory Care Unit. Requirements to apply:

• Graduation of accredited RN program
• Strong knowledge of MDS/OBRA system
• Excellent communications skills to interact with staff, families, and residents
• Ability to maintain assigned work hours
• Sufficient endurance to perform tasks over a long period of time

Core Responsibilities:
• Updates case managers per Managed Care contract guidelines and promoting positive communications with external agencies.
• Responsible for oversight of the Medicare and Managed care reimbursement in the facility. This position works with all departments to ensure regulatory compliance and positive clinical outcomes while maximizing reimbursement.
• Facilitates completion and deliverance of advanced beneficiary notices
• Oversight and coordination of Medicare certification process
• Monitoring of skilled nursing documentation and providing guidance and education to support skilled nursing level
• Active participation in the development of a comprehensive, individualized, interdisciplinary plan of care for each resident. Coordinating and facilitating interdisciplinary Resident Care Plan Conferences to meet state and federal regulations. Ensuring that the resident and responsible party receives written invitation and notification of the meeting, specifying time and place.
• Coordinates/attends all PPS and Weekly Medicare & Managed care meetings
• Maintaining a current knowledge base regarding state and federal regulations relative to the RAI process and PPS.
• Maintenance and coordination with IDT of PPS/OBRA MDS calendar
• Coordination of all required MDS/RAI per state and federal regulations to ensure timely completion.
• Review of each resident’s case on the day of admission/readmission to insure appropriateness of skilled coverage
• Timely transmission to CMS of all required MDS assessments. per state and federal regulations.
• ICD-10 coding: Initiate on admission of residents to facility and participation in maintenance of ongoing diagnosis list.
